Are you a health professional working with people who have heart failure?

This survey seeks to understand how palliative care is provided for patients with heart failure in your country. It is part of the RAPHAEL project, an international project developing the integration of heart failure and palliative care services in Europe, funded by the Horizon Europe program (#101137170). This project and survey have been developed by experts in cardiology, palliative and primary care, and has been approved by the UMC Utrecht ethics committee (24U-1483).
